Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Deliver compassionate physical therapy in patients' homes
Perform evaluations, OASIS assessments, and treatments
Develop personalized care plans to boost mobility and strength
Collaborate with healthcare teams for optimal outcomes
Document progress and adjust therapies for maximum impact
Travel within territory to serve diverse communities

Qualifications

Active CA PT License
BLS certification
Valid Driver's License with auto insurance and registration
Up-to-date immunizations including TB annual, MMR, Varicella, HepB, Tdap within 10 years, Flu annual
Clear drug screen and physical exam

Education

Physical Therapy degree from accredited program

Knowledge & Skills

Expertise in home health physical therapy
OASIS documentation proficiency
Patient evaluation and treatment planning
Strong communication for patient and physician interactions
Reliable driving and territory management
