Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop and implement individualized treatment plans for post-surgery, fall recovery, and other conditions
Collaborate with the interdisciplinary care team to ensure optimal patient outcomes
Educate patients and families on therapeutic exercises and home safety
Document patient progress accurately in HomeCare/HomeBase EMR
Travel within a 60-mile radius to deliver compassionate care

Qualifications

Current North Dakota Physical Therapy license
BLS certification required
Valid driver's license and auto insurance
Minimum 1 year of experience in physical therapy, with home health experience preferred
Must not have been employed with CommonSpirit (CHI/Dignity) in the past 12 months

Knowledge & Skills

Proficient in HomeCare/HomeBase EMR
Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and manage a caseload of 5-7 patients per day
Knowledge of home health regulations and infection control
