Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Conduct initial evaluations and develop personalized care plans
Perform SOC visits, especially for orthopedic and post-surgical patients
Collaborate with the care team to ensure optimal patient outcomes
Document patient progress accurately in HCHB EMR

Qualifications

Active Physical Therapist license in California
Home Health experience is required
BLS certification from the American Heart Association
Ability to travel up to 40 miles per day within Tehama County

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Proficiency with HomeCare HomeBase (HCHB) EMR
Excellent communication and patient education abilities
Experience with orthopedic, geriatric, CHF, and COPD patients
