Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Perform regular patient visits, averaging 5-6 per day, including admissions and assessments
Collaborate with clinical coordinators and sister agencies to ensure seamless care
Provide safety evaluations and occasional hospice teaching visits as needed
Document patient progress and maintain accurate records

Qualifications

Active North Dakota Physical Therapy License
Current BLS certification
Valid driver's license and auto insurance
Home health experience and OASIS training strongly preferred
Willing to accept first-time travelers with the right skills

Knowledge & Skills

Strong clinical skills in home health and hospice settings
Ability to independently manage patient caseload and prioritize visits
Excellent communication and collaboration with interdisciplinary teams
Proficiency with OASIS documentation and home health regulations
