Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturday rotation once per month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' therapeutic and rehabilitative needs
Develop and implement physical therapy plans of care based on physician orders
Instruct patients and families in therapeutic exercises and appliance use
Monitor patient progress and adjust treatment plans accordingly
Coordinate with other healthcare professionals for comprehensive care
Maintain accurate clinical documentation and progress summaries
Participate in case conferences and staff competency assessments

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Excellent oral and written communication skills
Strong organizational skills
Ability to work independently and as part of a team
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical therapy assessment and treatment techniques
Knowledge of Medicare, Medi-Cal, and Title 22 regulations
Home health and rehabilitation best practices
Patient and family education
Documentation and reporting skills
Time management and prioritization
