Contract Type: Travel Contract

Shift Type: Monday-Friday, Days (with weekend rotation approximately every 4th week)

Hours per Week: 36

Candidate Pay Rate: $67 per hour

Roles & Responsibilities

Provide individualized, goal-oriented nursing care in patients' homes using the nursing process
Serve as Case Manager for patients, coordinating care with physicians and multidisciplinary teams
Develop and update care plans, monitor patient progress, and educate patients and families
Administer medications, wound care, and other skilled nursing interventions
Document all care accurately and timely in electronic health records

Qualifications

Current and active Registered Nurse (RN) license in Vermont
Minimum of 1 year of home health or related nursing experience
Current BLS/BCLS certification (American Heart Association)
Strong clinical assessment, critical thinking, and communication skills

Knowledge & Skills

Expertise in home health nursing, case management, and patient education
Ability to work independently and prioritize care in a home setting
Familiarity with EHR systems and documentation standards
