Roles & Responsibilities

Provide individualized goal-oriented nursing care using the nursing process in accordance with physician orders
Serve as Case Manager when leading the discipline, coordinating comprehensive care plans
Deliver skilled home health and hospice care to patients in their homes, promoting independence and quality of life
Collaborate with an interdisciplinary team to ensure holistic, patient-centered care
Educate patients and families on disease management, medications, and home safety
Document patient progress and communicate effectively with healthcare providers

Qualifications

Active and unrestricted Vermont RN license required
Minimum 1 year of nursing experience, preferably in home health or hospice
Current BLS/BCLS certification required
Strong clinical assessment and critical thinking skills
Ability to work independently and manage a caseload
Reliable transportation and willingness to travel between patient homes

Knowledge & Skills

Knowledge of home health care principles and regulations
Case management and care coordination skills
Excellent communication and interpersonal abilities
Proficiency with electronic medical records (EMR)
Compassionate, patient-centered approach to care
Time management and organizational skills
