Shift Type: Monday-Friday, Days

Hours per Week: 40

Candidate Pay Rate: $84 per hour

Roles & Responsibilities

Collaborate with medical providers and interdisciplinary teams to establish individualized medical goals
Coordinate multi-disciplinary care conferences for high-risk or complex patients
Manage panel acute and chronic care needs by reviewing EHR reminders and providing outreach and education
Facilitate coordination of patient referrals and track progress of care
Perform staff nursing duties including assessment, documentation, education, and administering medications
Maintain annual competencies and adhere to infection control policies

Qualifications

Active Alaska RN license required
Minimum 1 year of recent Case Management experience
BLS certification required
Graduate of an accredited nursing program (BSN preferred)
National Case Management certification is a plus

Education

Graduate of an accredited school of nursing
Bachelor's degree in Nursing preferred

Knowledge & Skills

Knowledge of nursing process and case management principles
Strong assessment and data collection skills
Excellent communication and teamwork abilities
Ability to multitask in a fast-paced environment
Proficiency with computers and medical equipment
Problem-solving and conflict resolution skills
