Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy to patients in their homes, helping them regain mobility and independence.
Conduct start-of-care visits and develop personalized treatment plans.
Collaborate with a supportive interdisciplinary team to ensure comprehensive care.
Document patient progress using HCHB EMR.
Educate patients and families on home exercise programs and safety.

Qualifications

Current Physical Therapist license in California
BLS certification required
Minimum 1 year of home health experience (or less considered with strong home health background)
Experience with HCHB EMR preferred
Valid driver's license and reliable transportation

Knowledge & Skills

Home health care knowledge
Orthopedic rehabilitation and geriatric care expertise
Excellent communication and time management skills
Ability to travel within service area (approximately 40 miles per day)
Comfort with using a tablet for EMR
