Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide comprehensive physical therapy assessments and treatments to home health patients
Develop individualized care plans focusing on functional independence
Educate patients and families on therapeutic exercises and safety strategies
Collaborate with interdisciplinary team to ensure coordinated care
Maintain accurate documentation in HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver care in patient homes

Qualifications

Licensed Physical Therapist in the state of North Dakota
Current BLS certification (American Heart Association)
Valid driver's license and auto insurance
Minimum 1 year of recent physical therapy experience
Previous home health experience preferred
No previous travel experience required - we welcome first-time travelers

Knowledge & Skills

Expertise in home health physical therapy and patient assessment
Proficiency with HomeCare/HomeBase EMR software
Strong clinical reasoning and problem-solving abilities
Excellent communication and patient education skills
Ability to manage a caseload of 5-7 patients per day
Knowledge of treatment plans for post-surgery, falls, and chronic conditions
