Roles & Responsibilities

Conduct psychosocial assessments and develop comprehensive care plans for patients in crisis
Coordinate with interdisciplinary team to ensure seamless care and discharge planning
Provide crisis intervention and support for patients requiring 5150 evaluations and holds
Advocate for patients' needs and connect them with community resources
Document patient progress and maintain accurate clinical records
Collaborate with Dignity Health facilities to ensure continuity of care

Qualifications

Licensed Clinical Social Worker (LCSW) in California
Current BLS certification
5150 certified in California (training and exam completed)
Prior experience with Dignity Health or CHI facilities required
Strong clinical assessment and crisis intervention skills
Excellent communication and documentation abilities

Knowledge & Skills

Knowledge of psychiatric evaluation and 5150 holds
Care coordination and discharge planning expertise
Interdisciplinary collaboration skills
Understanding of mental health resources and community support
Proficiency in electronic health records
