Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturdays (once a month)

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in home health settings
Develop and update individualized treatment plans in collaboration with physicians
Educate patients and families on therapeutic exercises and equipment use
Monitor progress and adjust treatments to achieve functional goals
Collaborate with interdisciplinary team to coordinate comprehensive care
Maintain accurate clinical documentation and progress notes

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute care, home care, or rehab
Active California PT license required
BLS certification from AHA required
Excellent communication and organizational skills
Ability to work independently and manage a caseload across Yolo County

Knowledge & Skills

Expertise in physical therapy assessment and treatment planning
Proficiency in home health regulations and Medicare guidelines
Strong documentation and time management skills
Ability to educate patients and caregivers effectively
Knowledge of local community resources for patient referrals
