Licensed Practical Nurse (LPN) - Long Term Care

Location: Breckenridge, Minnesota

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Cycle scheduling with shifts available: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, and 10:00 PM - 6:30 AM. Weekend requirement: Every other weekend.

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate care to residents in a long-term care setting
Administer medications and treatments as prescribed
Monitor patient health and report changes to the healthcare team
Collaborate with CNAs and other staff to ensure quality care
Maintain accurate patient records using PointClickCare EMR
Support a home-like environment with strong teamwork ethics

Qualifications

Active LPN license in Minnesota (or compact state)
BLS certification required
Previous long-term care experience preferred
Strong communication and teamwork skills
Must meet the 50-mile radius rule (not a full-time/PRN employee with CommonSpirit in the past year)

Knowledge & Skills

Knowledge of LTC regulations and patient care standards
Skills in medication administration and IV therapy (minimal)
Ability to use PointClickCare EMR
Compassionate and patient-centered approach
Ability to work in a cycle scheduling environment
