Location: Santa Maria, California

Start Date: 06-18-2026

End Date: 09-17-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with flexibility; every 3rd weekend required

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Develop and implement individualized treatment plans to restore function and mobility
Collaborate with a multidisciplinary team including OTs, SLPs, and rehab aides
Complete thorough Oasis documentation and maintain accurate patient records in Cerner
Educate patients and families on home exercise programs and fall prevention
Cover assigned geographical region with a focus on patient-centered care

Qualifications

Active Physical Therapist license in California (or ability to obtain)
Current BLS certification from American Heart Association
Minimum one year of experience as a PT (home health experience preferred)
Experience with Oasis documentation required
Comfortable using Cerner EMR system
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health and community-based care knowledge
Skilled in Oasis assessment and documentation
Proficient with Cerner electronic medical records
Strong clinical reasoning and problem-solving abilities
Excellent communication and patient education skills
Ability to work independently and manage a flexible schedule
