Shift Type: Day shifts, Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial and ongoing assessments for home health patients
Develop and implement individualized care plans
Provide skilled physical therapy interventions to improve mobility and function
Educate patients and families on home exercise programs
Collaborate with interdisciplinary team including RNs and OTs
Document patient progress accurately in HCHB EMR

Qualifications

Active California Physical Therapy license
BLS certification from American Heart Association
Minimum of 2 years home health experience (or less with strong home health background considered)
Experience with HCHB EMR preferred
Valid driver's license and reliable transportation

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Strong clinical assessment skills
Knowledge of orthopedic and geriatric patient populations
Ability to work independently in a home health setting
Excellent communication and patient education skills
Proficiency in HomeCare HomeBase (HCHB) EMR
