Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (no weekends, no holidays)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized treatment plans
Provide skilled physical therapy services to home health patients, including orthopedic rehabilitation
Perform Start of Care (SOC) visits for patients, especially those with orthopedic conditions
Collaborate with an interdisciplinary team to ensure coordinated, high-quality care
Document patient progress in HomeCare HomeBase (HCHB) electronic medical records
Educate patients and families on therapeutic exercises and home safety

Qualifications

Active Physical Therapist (PT) license in California
Current BLS certification from the American Heart Association
Minimum 1 year of home health experience (new graduates considered with home health background)
Ability to travel within Tehama County service area (approximately 40 miles per day)

Knowledge & Skills

Strong patient assessment and clinical reasoning skills
Experience with orthopedic rehabilitation and geriatric patient populations
Proficient in HomeCare HomeBase (HCHB) EMR system
Excellent communication and patient education abilities
Ability to work independently in a home health setting
