Roles & Responsibilities

• Provide top-notch physical therapy services in patients' homes, helping them regain independence
• Perform comprehensive evaluations, re-evaluations, and develop individualized care plans
• Deliver skilled therapeutic interventions including therapeutic exercises, manual therapy, and patient education
• Collaborate with a multidisciplinary care team to ensure coordinated, high-quality care
• Document patient progress accurately and timely in the EMR system
• Participate in rotating weekend and holiday coverage to support continuity of care

Qualifications

• Current Physical Therapist (PT) license in the state of Kentucky
• Prefer experience in home health settings, but new grads are welcome to apply
• BLS certification required from the American Heart Association
• Strong communication and organizational skills
• Ability to work independently and manage a caseload within a defined territory
• Reliable transportation and willingness to travel to patient homes in and around Louisville

Education

• Graduate from an accredited physical therapy program (Doctorate or Master's degree required)

Knowledge & Skills

• Expertise in evaluating and treating orthopedic, neurological, and cardiopulmonary conditions
• Proficiency in developing and modifying treatment plans based on patient progress
• Knowledge of home health regulations and documentation standards
• Ability to educate patients and families on home exercise programs and safety
• Comfortable using a tablet for documentation and communication
