Candidate Pay Rate: $31 per hour

Roles & Responsibilities

Provide compassionate care to residents in a skilled nursing and rehabilitation setting
Administer medications and treatments as prescribed
Monitor vital signs and report changes to the healthcare team
Assist with daily living activities and promote patient independence
Collaborate with nurses and therapists to deliver comprehensive care
Maintain accurate documentation and uphold safety standards

Qualifications

Current Licensed Practical Nurse (LPN) license in New Hampshire
Basic Life Support (BLS) certification from AHA or equivalent
At least 1 year of LPN experience in a skilled nursing or rehab setting
Strong clinical assessment and communication skills
Ability to work collaboratively in a fast-paced environment

Education

Graduate of an accredited practical nursing program

Knowledge & Skills

Medication administration and IV therapy monitoring
Wound care and infection control protocols
Patient assessment and care planning
Electronic health records (EHR) proficiency
Compassionate bedside manner and patient advocacy
