Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Evaluate and treat patients with communication, cognitive, and swallowing disorders in home health settings
Develop and implement individualized treatment plans to achieve patient goals
Collaborate with physicians, nurses, and other therapists for coordinated care
Educate patients and families on home exercise programs and strategies
Document patient progress and maintain accurate clinical records
Travel to patient homes across a multiple-county territory (up to 2.5 hours across)

Qualifications

Active Kentucky Speech Language Pathologist license in hand at submission
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
Current BLS certification
Minimum 1 year of clinical experience
Home health experience required

Knowledge & Skills

Strong knowledge of home health regulations and best practices
Excellent communication and interpersonal skills
Ability to work independently and manage a caseload
Proficient with electronic documentation and telehealth tools
Compassionate and patient-centered approach to care
