Shift Type: Tuesday-Saturday, 8:00 AM - 5:00 PM (flexible schedule)

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide skilled nursing care to home health patients, including wound care, IV therapy, and patient education
Manage a caseload of approximately 5 visits per day, adjusting for higher-point start of care visits (2.5 points each) and recertifications
Conduct comprehensive assessments and develop individualized care plans for patients with CHF, COPD, wounds, and IV needs
Collaborate with home health aides and other team members to ensure coordinated care
Maintain accurate documentation in HomeCare HomeBase (HCHB) and communicate effectively with the clinical team

Qualifications

Current and valid RN license in California
BLS certification required
Valid driver's license and reliable transportation with proof of insurance
At least 1 year of home health experience preferred
Proficiency in HomeCare HomeBase (HCHB) strongly preferred

Knowledge & Skills

Strong clinical skills in wound care, wound vac management, IV therapy, and PICC line care
Excellent patient education and communication skills, especially for high-risk rehospitalization patients
Ability to work independently, prioritize tasks, and manage time effectively in a field setting
Knowledge of OASIS documentation and home health regulations
