Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend, rotating holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the Palliative Care team
Review patient charts and assess individual care needs
Facilitate Goals of Care discussions with patients and families
Provide education on disease process and advance care planning (Advance Directives, POLSTs)
Hold family meetings to coordinate care and offer support
Offer end-of-life discussions and emotional support to patients and loved ones
Attend daily rounds and collaborate with physicians, NPs, and fellow RNs
Serve patients throughout the hospital in a faith-based, compassionate environment

Qualifications

Active California RN license in good standing
2-5 years of recent experience in palliative care, hospice, or acute care setting
Current BLS certification from the American Heart Association
ICU or oncology experience is a plus
Must not have been employed by CommonSpirit/CHI/Dignity within the past year
Flexibility, strong organizational skills, and a desire to serve others

Knowledge & Skills

Expertise in palliative and hospice care principles
Strong acute care clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficiency with Cerner EMR system
Ability to navigate multiple hospital units and work collaboratively
Comfort with end-of-life discussions and family support
