Shift Type: Monday-Friday, Days (8:00 AM - 5:00 PM), no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide home health physical therapy services to a diverse patient population, including orthopedic joint replacements and geriatric patients with comorbidities like CHF and COPD
Perform start-of-care visits for ortho patients when nursing is not ordered
Manage an average daily census of 78, growing with PT availability
Complete 5 visits per day, covering Tehama County with mileage averaging 40 miles per day
Utilize HCHB (HomeCare HomeBase) EMR on a provided tablet for documentation
Collaborate with intake RN and manager in the office to coordinate patient care

Qualifications

Active Physical Therapist (PT) license in California
BLS certification required
Home health experience is required (candidates with less than 2 years experience considered with home health experience)
Strong communication and time management skills
Comfortable with daily travel and use of personal vehicle (mileage may vary)

Knowledge & Skills

Proficiency in HomeCare HomeBase (HCHB) EMR
Expertise in orthopedic rehab and geriatric care
Ability to manage multiple patient visits efficiently
Knowledge of home health regulations and documentation standards
Adaptability to a changing patient load and service area
