Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes across Noble, Lagrange, Steuben, and DeKalb counties
Conduct comprehensive Oasis Assessments and Start of Care visits for new patients
Develop and implement individualized treatment plans focusing on elderly patients with falls and fractures
Collaborate with interdisciplinary team to ensure coordinated patient care
Document patient progress and maintain accurate electronic health records using a tablet
Educate patients and families on home exercise programs and safety strategies

Qualifications

Active Physical Therapist license in Indiana (or compact eligibility)
Minimum 1 year of home health experience preferred
Valid driver's license and reliable transportation
Ability to travel within a multi-county service area
Excellent communication and time management skills

Knowledge & Skills

Proficiency in Oasis documentation and Start of Care processes
Strong clinical reasoning and assessment skills
Experience with elderly patient populations and fall prevention
Familiarity with point-based productivity systems
Ability to work independently and manage a flexible daily schedule
