Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients
Coordinate care and discharge planning with interdisciplinary teams
Provide crisis intervention and supportive counseling
Facilitate 5150 evaluations and involuntary holds when necessary
Collaborate with community resources to ensure smooth transitions
Advocate for patients' needs and rights throughout their care

Qualifications

Active California LCSW license
Current BLS certification
5150 certification required (training and exam with 80% pass rate)
Prior experience in a California Dignity Health facility is a must
Strong clinical assessment and crisis management skills

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Excellent psychosocial assessment and diagnostic skills
Knowledge of California laws regarding involuntary holds (5150)
Strong communication and interpersonal abilities
Ability to work collaboratively in a fast-paced environment
Proficiency in electronic health records (EHR) and documentation
