Contract Type: Travel Contract

Shift Type: Rotating schedule including weekends, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive evaluations and develop personalized treatment plans for patients in home health settings
Perform start of care, recertifications, and discharge visits with precision and compassion
Collaborate with clinical managers and educators to ensure optimal patient outcomes
Document patient progress and adjust treatments as needed
Educate patients and families on therapeutic exercises and home safety

Qualifications

Active Physical Therapist license in Kentucky or compact eligibility
BLS certification required
Prefer at least one year of experience, but new grads with strong clinical rotations are welcome
Home health experience a plus, but training and mentorship provided

Knowledge & Skills

Strong clinical reasoning and ability to manage diverse diagnoses like CHF, ortho, and pneumonia
Excellent communication and interpersonal skills
Comfortable with electronic documentation and telehealth tools
Ability to work independently in the field with support from a 24/7 clinical team
