Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents
• Administer medications and treatments safely and accurately
• Monitor patient status and promptly report changes to the care team
• Document care thoroughly and maintain accurate electronic health records
• Collaborate with RNs, physicians, therapists and support staff for coordinated care
• Support admissions, discharges, and transitions to ensure continuity
• Educate patients and families about care plans, medications and follow-up

Qualifications

• Active LPN license (Licensed Practical Nurse) or eligibility for New Mexico licensure
• Experience in long-term care, skilled nursing, or similar setting preferred
• Basic Life Support (BLS/CPR) certification required or obtainable upon hire
• Comfortable with medication administration, wound care, and routine assessments
• Strong communication skills and ability to work as part of a multidisciplinary team
• Current immunizations and background checks per facility policy

Education

• Completion of an accredited Practical Nursing program (LPN/LVN certificate or diploma)

Knowledge & Skills

• Thorough knowledge of LPN scope of practice and nursing fundamentals
• Proficient in medication administration, documentation, and infection control
• Skilled in vital signs assessment, wound care, and patient observation
• Able to use electronic health record systems for charting and care coordination
• Good clinical judgment, time management, and prioritization skills
