Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care to residents, ensuring dignity and comfort
Administer medications and treatments accurately according to care plans
Monitor, assess, and document changes in patient condition in a timely manner
Collaborate with RNs, CNAs, and the interdisciplinary team to coordinate care
Assist residents with activities of daily living and promote independence
Communicate clearly with residents and families about care and progress
Participate in infection control, safety protocols, and quality improvement initiatives
Maintain accurate and professional electronic and paper documentation

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania
Graduate of an accredited practical nursing program
Current CPR/BLS certification (or willingness to obtain)
Comfortable working in a long term care or skilled nursing environment
Positive attitude, strong communication skills, and ability to work as part of a team
Willingness to complete required background checks, health screening, and vaccinations

Education

Graduate of an accredited practical nursing program; LPN license required

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and ADL support
Patient assessment and vital signs monitoring
Infection control and safety procedures
Experience with electronic health record charting
Strong interpersonal skills for resident and family interactions
