Roles & Responsibilities

• Evaluate and assess residents' speech, language, cognitive-communication, and swallowing function
• Develop individualized, evidence-based treatment plans to improve communication and safety
• Deliver skilled therapy interventions and adapt techniques to each resident's goals
• Collaborate closely with nursing, therapy colleagues, and family members to coordinate care
• Document progress and treatment clearly and timely to meet facility and regulatory standards
• Educate residents and caregivers on strategies to maximize communication and swallowing safety

Qualifications

• Certificate of Clinical Competence (CCC-SLP) required
• 1-3 years of clinical experience (as listed in the job details)
• Experience working in a skilled nursing facility (SNF) setting preferred
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork skills; comfortable collaborating in an interdisciplinary setting
• Available for daytime schedules as required by the facility

Knowledge & Skills

• Skilled in dysphagia screening and management
• Experience with speech, language, and cognitive-communication assessments
• Proficient clinical documentation and care planning
• Ability to provide patient- and family-centered education
• Comfort working within EMR systems and following facility protocols
