Roles & Responsibilities

• Perform skilled occupational therapy evaluations to identify functional needs and goals
• Develop individualized treatment plans that promote independence and quality of life
• Deliver evidence-based OT interventions in a skilled nursing facility setting
• Document patient progress clearly and timely using facility charting standards
• Collaborate with nurses, PT/SLP, and interdisciplinary team members for coordinated care
• Educate residents, families, and staff on adaptive strategies and safe functional approaches
• Participate in discharge planning and transitions of care to ensure continuity
• Maintain professional standards, compliance, and infection control practices

Qualifications

• Active Occupational Therapy license in Indiana (or eligibility to obtain one)
• Any experience level considered, including new graduates
• Valid CPR/BLS certification (facility may require upon hire)
• Strong clinical reasoning and documentation skills
• Comfort working in a skilled nursing facility (SNF) environment
• Reliable, professional, and team-oriented approach to resident care

Education

• Degree in Occupational Therapy (entry-level degrees accepted; new grads welcome)

Knowledge & Skills

• Skilled in OT evaluation and treatment planning for geriatric and long-term care populations
• Proficient with documentation and SOAP-style progress notes
• Knowledge of adaptive equipment, splinting basics, and ADL retraining
• Ability to collaborate within an interdisciplinary team
• Solid clinical decision-making and time management skills
• Understanding of SNF workflows, Medicare/insurance basics, and compliance expectations
