Roles & Responsibilities

• Provide compassionate, evidence-based occupational therapy services to residents in a skilled nursing setting
• Perform thorough assessments and develop individualized treatment plans that help patients regain independence
• Collaborate with nursing, PT, SLP, and facility staff to coordinate care and optimize outcomes
• Document progress and outcomes accurately and timely in the facility charting system
• Adapt interventions for a variety of diagnoses commonly seen in SNF populations
• Float between Elk River Winchester and Fayetteville locations as needed, maintaining continuity of care

Qualifications

• Occupational Therapy credential (OTR/OTA) or equivalent — state licensure as required (or eligible to obtain licensure)
• Comfortable working in a Skilled Nursing Facility environment
• Willingness to float between Elk River Winchester and Fayetteville facilities
• Open to all experience levels, including New Grad clinicians
• Strong communication skills and ability to collaborate effectively with interdisciplinary teams

Education

• Open to clinicians at all levels; new graduates welcome
• Formal OT education required (specific degree not listed in job posting)

Knowledge & Skills

• Skilled in OT assessment and treatment planning for SNF residents
• Experience or comfort with skilled nursing workflows and documentation
• Ability to prioritize caseloads and manage time efficiently while floating
• Good clinical reasoning and adaptive problem solving
• Strong interpersonal skills for patient and team interactions
