Roles & Responsibilities

• Provide patient-centered occupational therapy services in a skilled nursing setting
• Assess functional needs and develop individualized treatment plans
• Deliver skilled interventions to help residents regain independence and safety
• Document care thoroughly and in accordance with CRG policies and facility procedures
• Collaborate with nursing and interdisciplinary team members to coordinate care
• Meet productivity expectations (90%) while maintaining high-quality therapy
• Represent the therapy team professionally and adhere to facility dress code

Qualifications

• 1-3 years of occupational therapy experience (as listed in the job notes)
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment
• Current/valid Occupational Therapist license as required by state regulations
• Strong documentation skills and ability to meet productivity requirements
• Ability to work onsite consistently to receive guaranteed hours
• Comfort following company policies and procedures (CRG P & P)
