Contract Type: Per-Diem

Shift Type: Per diem, 6:00 AM - 6:30 PM day shift (scheduled as needed)

Hours per Week: 38

Candidate Pay Rate: $30 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents at Belen Meadows Center
Administer medications and treatments accurately and document care thoroughly
Assess patients regularly and report changes in condition to the care team
Collaborate with interdisciplinary staff to develop and follow individualized care plans
Maintain clear, timely documentation in the patient record and ensure regulatory compliance
Educate patients and families about care plans, medications, and follow-up needs
Adhere to infection control and safety protocols to protect patients and staff

Qualifications

Active LPN or RN licensure (class listed as LPN | RN); state requirements apply
Willingness and availability to work per diem shifts (scheduled as needed)
Current Basic Life Support (BLS) or CPR certification as required by facility
Strong communication, assessment, and clinical documentation skills
Ability to work collaboratively in a fast-paced long-term care setting

Education

LPN: Completion of an accredited practical nursing program or equivalent
RN: ADN or BSN from an accredited nursing program (if applying as RN)

Knowledge & Skills

Patient assessment and monitoring
Medication administration and safe handling
Wound care basics and pressure injury prevention
Electronic health record documentation and charting
Strong clinical judgment and team communication
