Roles & Responsibilities

• Provide compassionate bedside care and assist with activities of daily living for residents
• Administer medications and treatments safely and document findings accurately
• Monitor patient status, recognize changes, and escalate concerns to the care team
• Collaborate with nurses and interdisciplinary staff to support individualized care plans
• Maintain clear, timely documentation in patient charts and electronic medical records
• Support admission, discharge, and transfer processes to ensure smooth transitions

Qualifications

• Active Licensed Practical Nurse (LPN) license
• Experience working in long-term care or similar clinical settings is a plus
• Current Basic Life Support (BLS) certification preferred
• Ability to pass standard background and health screenings required by the facility
• Strong communication and teamwork skills to work well with patients and staff

Education

• Practical Nursing diploma or certificate (or equivalent) required

Knowledge & Skills

• Skilled in medication administration and basic treatments
• Comfortable performing patient assessments and vital sign monitoring
• Proficient with charting and maintaining accurate patient records
• Knowledgeable about infection control and resident safety practices
• Excellent interpersonal skills for working with diverse patient populations
• Able to follow care plans and collaborate effectively with interdisciplinary teams
