Roles & Responsibilities

• Provide compassionate, hands-on patient care and assistance with activities of daily living
• Assist patients with bathing, dressing, grooming, toileting, and mobility
• Take and accurately record vital signs and report changes to the nursing team
• Support safe transfers and fall-prevention measures
• Document patient observations and care given in a timely, professional manner
• Communicate clearly and kindly with patients, families, and interdisciplinary team members
• Maintain a clean, safe environment and adhere to infection control practices

Qualifications

• Current CNA certification (must be active and in good standing)
• Required state certification or ability to obtain Pennsylvania CNA certification
• Basic Life Support (BLS) / CPR preferred or willing to obtain
• Up-to-date immunizations and TB test per facility policy
• Clear background check and ability to pass facility onboarding requirements
• Compassionate attitude, strong communication skills, and reliability

Education

• High school diploma or GED required
• Current CNA certification required

Knowledge & Skills

• Skilled in providing ADL assistance and basic bedside care
• Able to take and accurately record vital signs and patient observations
• Familiarity with infection control and safety protocols
• Comfortable with basic resident lifting/transfers and use of assistive devices
• Good documentation and basic charting skills
• Team-oriented communication and the ability to follow care plans
