Roles & Responsibilities

Coordinate comprehensive patient care and discharge planning in a hospital setting
Provide psychosocial assessments and crisis intervention for patients and families
Facilitate 5150 holds for involuntary psychiatric evaluations as needed
Collaborate with interdisciplinary teams to ensure safe patient transitions
Document all interactions and interventions accurately in medical records
Champion patient rights and connect individuals to community resources

Qualifications

Current California LCSW license required
BLS certification from American Heart Association
5150 Certification in California (completed training program and passed exam with 80% or higher)
Prior experience with Dignity Health or CommonSpirit facilities highly preferred
Two supervisor references (charges accepted) - one within 12 months, one within 3 years of worked dates
Must not have been directly employed by any CommonSpirit/Dignity/CHI facility within the past year

Knowledge & Skills

Strong assessment and diagnostic skills for mental health conditions
Expertise in involuntary psychiatric holds (5150) and legal requirements
Excellent communication and interpersonal skills for patient and family advocacy
Care coordination and discharge planning in acute care settings
Proficiency with electronic medical records and documentation
