Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide high-quality physical therapy services to patients in their homes across four counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Develop and implement personalized treatment plans for elderly patients recovering from falls and fractures
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Document patient progress accurately using tablet-based system
Educate patients and families on therapeutic exercises and safety strategies

Qualifications

Active Physical Therapist (PT) license in Indiana
Experience with Oasis Assessments and Start of Care cases required
Comfortable working in rural and mid-sized town settings
Valid driver's license and reliable transportation for home visits
Ability to manage a caseload of 4-6 patient visits daily

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Proficient with electronic documentation and tablet use
Knowledge of home health regulations and Medicare guidelines
