Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotation once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in their home environment
Develop and implement personalized treatment plans to improve mobility and function
Educate patients and families on therapeutic exercises and use of equipment
Monitor progress and adjust care plans in collaboration with the physician
Document clinical notes and communicate effectively with the healthcare team
Supervise home health aides and participate in case conferences

Qualifications

Minimum of 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification (required)
Valid Physical Therapy license in California (or ability to obtain before start)
Excellent communication and organizational skills
Comfortable driving to multiple patient locations in Yolo County
Ability to work independently and manage a flexible schedule

Knowledge & Skills

Home health and rehabilitation experience
Patient assessment and treatment planning
Prosthetic device training (desired)
Knowledge of Title 22, Medicare Conditions of Participation, and OSHA regulations
Strong interpersonal skills to connect with patients and families
