Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes
Develop and implement individualized treatment plans
Treat patients recovering from surgeries, falls, and other conditions
Collaborate with the home health team to ensure comprehensive care
Document patient progress using HomeCare/HomeBase EMR

Qualifications

Active North Dakota Physical Therapy License
BLS certification required
Minimum 1 year of physical therapy experience
Previous home health experience strongly preferred
Valid driver's license and auto insurance required

Knowledge & Skills

Proficient in HomeCare/HomeBase electronic medical records
Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to travel within a 60-mile radius of Valley City
