Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (no weekends or holidays)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial assessments and start-of-care visits for orthopedic and geriatric patients
Develop and implement individualized physical therapy plans in home settings
Educate patients and families on therapeutic exercises and fall prevention
Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated care
Document patient progress using HCHB EMR system on provided tablet

Qualifications

Active Physical Therapy license in California
Current BLS certification (American Heart Association)
Home health experience required; new grads with home health clinicals considered
Comfortable traveling up to 40 miles per day within Tehama County
Must be able to manage a caseload of 5 visits per day

Knowledge & Skills

Proficiency in HomeCare HomeBase (HCHB) EMR
Experience with orthopedic post-surgical and geriatric co-morbidities (CHF, COPD)
Excellent clinical judgment and communication skills
Ability to work independently and manage time effectively
