Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Develop and implement personalized physical therapy plans
Perform OASIS assessments and documentation
Collaborate with clinical team and coordinate patient care
Educate patients and families on therapy exercises and safety

Qualifications

Active Physical Therapy license in North Dakota
Current BLS certification
Valid driver's license and auto insurance
Home health experience preferred
OASIS training preferred
Willingness to travel within 50-mile radius

Knowledge & Skills

Strong clinical assessment and problem-solving skills
Proficiency in OASIS documentation
Excellent communication and patient education abilities
Time management and independent work
Adaptability to changing patient needs
