Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments as prescribed, ensuring safety and accuracy
• Monitor vital signs, document patient progress, and report changes to the care team
• Collaborate with CNAs, RNs, and interdisciplinary team to maintain a home-like environment
• Champion infection control, fall prevention, and quality improvement initiatives
• Educate residents and families on health management and care plans

Qualifications

• Active LPN license in Minnesota (or compact state eligible)
• BLS certification from American Heart Association required
• Minimum of 1 year LPN experience in long-term care or skilled nursing
• Comfortable with minimal IV medication administration
• Strong teamwork and communication skills
• Ability to pass background check and drug screen

Knowledge & Skills

• Proficiency in PointClickCare EMR system
• Knowledge of CHF, COPD, Alzheimer's, Parkinson's, and diabetes management
• Understanding of LTC regulations and survey readiness
• Excellent prioritization and time management
• Compassionate approach to end-of-life and dementia care
