Roles & Responsibilities

• Evaluate patients thoroughly and create individualized treatment plans that lead to measurable improvements — your clinical reasoning matters.
• Provide skilled physical therapy interventions across inpatient, swing bed, outpatient, and skilled nursing settings in accordance with physician orders and regulations.
• Document patient status, progress, and plan of care accurately and promptly in the EMR to ensure continuity and quality of care.
• Collaborate with physicians, nurses, and the interdisciplinary team to coordinate care and optimize patient outcomes.
• Educate patients and families on treatment goals, home programs, and safe mobility strategies to promote independence.
• Participate in quality improvement and professional development activities to keep skills sharp and care evidence-based.

Qualifications

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA BLS (Basic Life Support) certification
• Minimum 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with computers and EMR systems; strong documentation skills

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Strong clinical reasoning and assessment skills for diverse patient populations
• Effective verbal and written communication with patients and the care team
• Accurate and timely documentation in EMR systems
• Problem solving and attention to detail to manage complex caseloads
• Ability to adapt treatment approaches across inpatient, swing bed, outpatient, and skilled nursing environments
