Roles & Responsibilities

Provide high-quality outpatient family medicine care for patients across the lifespan
Build long-term relationships with patients and families, delivering patient-centered care
Manage acute and chronic conditions with evidence-based approaches
Collaborate with experienced clinical and administrative staff to coordinate care
Participate in community health initiatives and help shape local health priorities
Maintain accurate medical records and ensure compliance with clinical policies

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
Commitment to community-focused and rural healthcare
Strong interpersonal skills and ability to work in a collaborative team environment

Education

MD or DO from an accredited medical school; completion of Family Medicine residency required

Knowledge & Skills

Outpatient family medicine and primary care across the lifespan
Chronic disease management and preventive care
Excellent clinical judgment and patient-centered communication
Ability to work effectively in a small, community hospital setting
Comfortable with common ambulatory EMR systems and documentation practices
Team collaboration and care coordination skills
