Roles & Responsibilities

• Assess and treat patients with movement dysfunction or pain to improve daily function and independence
• Develop individualized treatment plans focused on pain management, fall risk reduction, and mobility improvement
• Document evaluations, progress notes, daily notes, discharges, and insurance approvals/denials accurately and timely
• Collaborate with other providers, rehab staff, and families through clear phone and in-person communication
• Assist with discharge planning to ensure safe transitions and positive patient outcomes
• Support clinic operations including light cleaning of treatment rooms, assisting with laundry, and maintaining a safe environment
• Perform other duties as requested to support patient care and the therapy team

Qualifications

• Current Physical Therapist license in Wisconsin or eligibility to obtain licensure quickly
• DPT or Master's in Physical Therapy preferred
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfortable with documentation systems and writing clear clinical evaluations and progress notes
• Experience or demonstrated skills in pain management, fall risk assessment, and mobility training
• Strong communication skills for coordination with patients, families, providers, and insurers
• Ability to assist with light housekeeping tasks and work collaboratively within a team

Education

• DPT (Doctor of Physical Therapy) or Master's in Physical Therapy preferred; state licensure required

• Fall risk assessment and prevention strategies
• Clinical evaluation and progress note documentation
