Roles & Responsibilities

• Provide compassionate direct patient care, including hygiene, vital signs, and basic treatments
• Administer medications and document administration according to facility policy
• Perform focused patient assessments and report changes in condition promptly
• Collaborate with RNs and interdisciplinary team members to develop and implement care plans
• Maintain accurate, timely documentation in the patient record
• Support residents and families with clear communication and empathy
• Follow infection control, safety protocols, and facility policies to ensure a safe environment
• Participate in shift handoffs and contribute to a positive team culture

Qualifications

• Current and active LPN license in Pennsylvania (required)
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Current immunizations and TB screening per facility policy
• Strong communication skills and a team-oriented mindset
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Reliable, punctual, and committed to quality patient care

Education

• Graduation from an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Knowledge of medication administration and documentation best practices
• Skilled in taking and interpreting vital signs and basic assessments
• Familiarity with electronic health records (EHR) or willingness to learn
• Ability to prioritize tasks and manage time effectively during a busy shift
• Strong interpersonal skills for patient and family interactions
• Understanding of infection prevention and safety standards
