Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents/patients
• Administer medications and treatments according to care plans and policies
• Monitor patient condition and report changes promptly to RN/leadership
• Document care accurately and legibly in the clinical record
• Support admissions, transfers, and discharges with professionalism
• Collaborate with the interdisciplinary team to promote quality outcomes
• Assist with activities of daily living and promote patient dignity and comfort
• Participate in safety, infection control, and quality improvement initiatives

Qualifications

• Valid LPN license (PA preferred or eligible to obtain PA licensure)
• Completion of an accredited practical nursing program
• Current BLS/CPR certification or willingness to obtain upon hire
• Ability to pass background check and drug screening as required
• Strong communication skills and a team-first attitude
• Previous LTC/skilled nursing experience a plus (but all qualified applicants encouraged to apply)

Education

• Completion of an accredited Practical Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital sign monitoring
• Wound care, infection control, and safe patient handling
• Familiarity with electronic medical records and charting
• Ability to prioritize tasks in a busy clinical environment
• Strong interpersonal skills for patient and family interactions
