Roles & Responsibilities

• Provide compassionate direct patient care to residents and patients
• Administer medications and treatments safely and accurately
• Monitor and document patient vital signs, status changes, and interventions
• Collaborate with nurses and the interdisciplinary team to implement care plans
• Educate patients and families about care plans, medications, and follow-up
• Maintain accurate and timely electronic documentation and charts
• Adhere to infection control, safety protocols, and facility policies
• Support admission, discharge, and transfer processes to ensure continuity of care

Qualifications

• Current LPN (Licensed Practical Nurse) license (New Hampshire license preferred)
• BLS/CPR certification required or willingness to obtain
• Ability to pass background check and drug screen
• Strong communication and teamwork skills
• Prior experience in long-term care, skilled nursing, or similar setting preferred

Education

• Graduate of an accredited LPN/practical nursing program or equivalent

Knowledge & Skills

• Medication administration and basic pharmacology
• Clinical assessment, vital signs, and patient monitoring
• Wound care and basic nursing procedures
• Accurate electronic medical record charting
• Patient-centered care, safety, and infection control practices
• Time management and prioritization in a fast-paced environment
