Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care as an LPN to residents
Administer medications and treatments accurately and safely
Monitor and document vital signs, patient status and changes in condition
Support care plan implementation and communicate observations to the care team
Assist residents with activities of daily living while preserving dignity and independence
Maintain accurate, timely electronic and paper documentation
Follow infection control and safety protocols to protect residents and staff
Collaborate with nurses, therapists and leadership to deliver high-quality care

Qualifications

Current LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program
BLS/CPR certification (current or obtained upon hire)
Familiarity with medication administration and basic wound care
Strong communication, documentation and teamwork skills
Ability to pass background check and meet facility immunization/TB policies

Education

Graduate of an accredited practical nursing (LPN) program; high school diploma or GED required

Knowledge & Skills

Clinical assessment and basic nursing interventions
Medication administration and charting best practices
Understanding of infection prevention and patient safety
Comfortable working in a long-term care/skilled nursing environment
Effective interpersonal skills and the ability to work in interdisciplinary teams
Time management and prioritization during busy shifts
