Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and timely
• Monitor and record patient conditions, reporting changes to the RN or provider
• Assist with activities of daily living to preserve dignity and independence
• Collaborate with the care team to create and follow individualized care plans
• Maintain clear, accurate documentation and charting
• Support a safe, clean environment and follow infection control practices
• Build strong relationships with patients, families, and coworkers

Qualifications

• Current LPN (Licensed Practical Nurse) license in Pennsylvania
• Completion of an accredited LPN training program
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Ability to pass background check and drug screening as required
• Strong communication and teamwork skills; patient-focused attitude
• Reliable, punctual, and able to handle the physical demands of nursing

Education

• High school diploma or equivalent AND graduation from an accredited LPN program

Knowledge & Skills

• Knowledge of medication administration and basic nursing procedures
• Familiarity with electronic charting and documentation best practices
• Understanding of infection control and patient safety protocols
• Skill in patient assessment, vital signs, and reporting changes
• Ability to prioritize tasks in a fast-paced healthcare setting
• Compassionate bedside manner and strong interpersonal skills
