Roles & Responsibilities

• Provide compassionate, patient-centered nursing care with attention to dignity and safety
• Administer medications and treatments accurately and document per facility policy
• Monitor patient status and report changes promptly to the care team
• Collaborate with nurses, CNAs, therapists, and physicians to coordinate excellent care
• Maintain accurate, timely clinical documentation and support continuity of care
• Educate patients and families about care plans, medications, and discharge instructions

Qualifications

• Current LPN license (practical/vocational nursing) — ability to practice in Pennsylvania preferred
• Recent clinical experience as an LPN or equivalent practical nursing experience
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Strong communication skills and the ability to work collaboratively with interdisciplinary teams
• Reliable professional references and a commitment to quality patient care

Education

• Graduate of an accredited practical/vocational nursing program (LPN/LVN diploma)

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Familiarity with patient charting and electronic health records
• Ability to assess patient conditions and escalate concerns appropriately
• Skilled at time management, prioritization, and working in a fast-paced environment
• Compassionate bedside manner and strong patient advocacy
